JOB DESCRIPTION SUMMARY

Responsible for coordinating hospice services for managed care patients which includes establishing and maintaining the patient's plan of care, establishing and maintaining positive relationships with managed care customers and related referral sources, and negotiating service agreements.

Service Area: LA County, San Bernandino County, Orange County, San Fernando Valley, Victorville and surrounding Areas

We are seeking a registered nurse to join our team of medical professionals. The successful candidate will be responsible for providing quality care to patients in hospice settings. The ideal candidate should have a valid RN license, communication skills, and the ability to work independently and as part of a team. This is an excellent opportunity for an experienced RN to make a difference in the lives of patients and their families.

Responsibilities:

  • Participation in the information gathering, analysis, education and form completion aspects of the admissions process.
  • Evaluate patients for hospice appropriateness and uses health assessment data to determine diagnosis.
  • Completes an initial assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es).
  • Recertification of patients
  • PRN Visits if needed
  • Patient assessments
  • Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition within appropriate time.
  • Knowledge of the principles of the intake, admission, and transition to care process.
  • Evaluate patients for hospice appropriateness and uses health assessment data to determine diagnosis.
  • Inform the patient of instructions for medication prescribed by a physician.
  • Take and asses vital signs, record patient information, and report changes to the team of physicians.
  • Provide emotional support to patients and their families.
  • Provide simple wound care if needed.
  • Maintain patient records in accordance with established standards.
  • PRN visits if necessary.
  • Medication reconciliation.
  • Checking the overall well-being of patients and any care they may require.
  • Coordinates hospice services for managed care patients, assists with the establishment of the plan of care in coordination with the physician and insurance and ensures a seamless transition to hospice.
  • Other assigned duties if necessary

POSITION QUALIFICATIONS

  • Prior Hospice experience-1 year (Required)
  • Must be a Registered nurse with current licensure to practice professional nursing in the state Graduate of National League for Nursing accredited school of nursing.
  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
  • Complies with acceptable professional standards and practice.
  • Ability to market aggressively and deal tactfully with customers and the community.
  • Knowledge of corporate business management and managed care environment.
  • Shows ability to organize and prioritize workload independently.
  • Demonstrates good communications, negotiation, and public relations skills.
  • Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
  • Prolonged or considerable walking or standing. Able to lift, position, and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, and/or crouching. Visual acuity and hearing to perform required nursing skills.

Job Types: Part-time, Per diem

Pay: $60.00 - $75.00 per hour

Expected hours: No more than 20 per week
